When I delete items from the Emule download queue, I am always asked "are you sure you want to cancel and delete this file", which requires a manual "yes" selection involving several keystrokes.

Is it possible to disable the delete confirmation so pressing delete just deletes the file without a dialog?

Hello, at the moment no, you can add a feature request here if you like this feature to be implemented.
Anyway a simple workaround is to select all the files you want to delete at a time and delete them all together, you can use the Ctrl or Shift buttons to add a single file to the selection or a range of files respectively.
In example if you have seven files in your Transfers window and you want to delete files number 1, 3, 5, 6 and 7 then click on the first file to select it, press Ctrl and select the file number 3, repeat for file number 5, then keep the Ctrl button hold pressed and press Shift button, select file number 7 to select all files from 5 to 7 while file number 1 and 3 are still selected, this way you have to click just one time on the red x in the toolbar and one click on the confirmation window to delete 5 files.
