I have looked into Xtreme and Dreamule but what Im wanting is the option like an edonkey mod had where you can either auto or manuel select a user in your queue and start their download.

i.e. if someone starts me I want them to start if they are in my queue, I think xtreme has add friend slot but I have to friend every time and create this

or often I see someone that I feel like starting because of a particular file they are after but cant or at least don't know how.

I've seen that but really wanted to just select a user to start, I may not even be in their queue so there wouldn't be a credit exchange correct?

There is no automated system like that. Be on their queue or not, if they had uploaded to you, credit system will reward them. (lovelace is more aggressive)
About unlimited friends slot, check Morph and Morph based mods. You can even determine friend slot bandwidth limit. (%100 for example, your whole upload goes to FS, but you'll get 1:3 download ratio because of that)
